Immunization Program
Immunization clinics are held Monday through Friday by appointment. Numerous types of vaccines are available depending on age and qualifications including uninsured and under-insured. Limited vaccine is available for children with insurance coverage and for those over 19 years old. Call 815-284-3371 Ext: 2124 for information. Medicaid and most private insurances are accepted.
High Risk Follow Up Program
Infants referred through APORS (Adverse Pregnancy Outcome Reporting System) are provided with home visits on a periodic basis. A physical and developmental assessment is completed. Referrals to other service providers are also made. Call 815-284-3371 Ext: 2122 for more information.
Family Case Management
Case management services are provided at no cost to pregnant women and infants under the age of one year who meet specific income guidelines.
